367 Improving Handover Documentation in Surgical Inpatients Using Weekend Handover Stickers

Abstract Introduction Transfer to the care oncoming team is point at which inpatients are most vulnerable. Effective handover vital protect patient safety and clinical governance. The Royal College of Surgery (RCS) Safe Handover guidelines highlight relevant information required for comprehensive handover. This QI project aimed determine if implementation weekend stickers in surgical patients’ notes improved documentation. Method A retrospective records-based audit patients admitted under general surgery over a 4-week period was performed. Standards were set using RCS sufficient documented. We designed be inserted based on guidelines. re-audit cycle performed intervention Results 119 initial audit. 125 re-audit. Documented from 43.7% 89.6% after intervention. Documentation following also improved: situation (43.7% 92.0%); co-morbidities (14.3% 89.6%); current issues (71.4% 94.4%); blood tests (32.8% antibiotic therapy (21.8% 92.0%). Conclusions Weekend resulted significant improvement documentation improving safety.
